What this website does and does not collect.
This site serves informational pages, and the hosting platform may create ordinary server access logs. The site does not create user accounts, run marketing analytics, or accept uploads.
The optional secure contact form collects a full name, phone number, optional email address, optional general message, preferred callback window, and acknowledgment. It does not ask for symptoms, diagnosis, medication information, medical history, date of birth, insurance information, records, uploads, attachments, signatures, or appointment dates.
How the secure contact request is handled.
The secure contact form is hosted and processed by Paubox rather than Gates Brain Health's website host. Paubox stores the submission and notifies info@gatesbrainhealth.com. Gates Brain Health has a 90-day retention target, but Paubox automatic retention and individual-deletion controls have not been verified; the target should not be read as implemented platform behavior.
Submitting the form does not establish, schedule, or confirm an appointment and does not guarantee a response time. The optional message is strictly for general, non-medical questions; do not include personal health information.
This privacy statement is not a medical-records channel.
Please call the office to ask how to share records or health history through an appropriate channel. Do not send those details in the secure contact form or ordinary email.
